The Standards of Care Program

The Standards of Care plan adopted by the Homeless Coalition of Hillsborough County establishes guidelines and definitions to ensure consistent and equal treatment of people accessing programs and services that are part of the Continuum of Care.  The Continuum is the consolidated and integrated housing and services delivery system that exists for individuals and families who are homeless.

 

 

 

Our Standards of Care plan establishes:

 

  • Individual rights of persons receiving services through the Continuum of Care

  • Defines who is and is not Homeless Coalition of Hillsborough County

  • Documentation requirements needed to access services based on a person or family’s situation, and

  • The purpose of and service standards for:

 

  • Prevention services

 

  • Street outreach

 

  • Emergency/temporary housing (shelter) facilities

 

  • Safe Havens

 

  • Transitional housing services

 

  • Permanent supportive housing for persons with disabilities

 

  • Linkages and referral supportive services
